Pancreatin- a pancreatic enzyme that takes part in the digestion of proteins, fats and carbohydrates in digestive tract. The decisive factor is the enzymatic activity of lipase, as well as the trypsin content, while amylolytic activity is important only in the treatment of cystic fibrosis, since even with a significant decrease in the secretory activity of the pancreas, amylase has sufficient activity. The coating covering the tablets is insoluble under the influence of gastric juice and protects enzymes from their inactivation gastric juice. Only under the influence of neutral or slightly alkaline environment small intestine, the membrane dissolves and enzymes are released.

Indications

  • Diseases accompanied by disturbances in the digestion of food due to insufficient excretion digestive enzymes pancreas, such as chronic pancreatitis, cystic fibrosis.
  • Conditions after simultaneous resection of the stomach and small intestine, functional acceleration of food passage through the intestines, intestinal disorders, simultaneous use difficult to digest vegetable, fatty and unusual foods.
  • Intestinal bloating and preparation for x-ray or ultrasound diagnostic studies.

Contraindications

Increased sensitivity to porcine pancreatin, azorubine varnish or other excipients drug. Acute pancreatitis or chronic pancreatitis in the acute stage (but occasional use is possible in the phase of fading exacerbation when expanding the diet in the presence of impaired pancreatic function). The drug should not be used in patients with obstructive intestinal obstruction.

Proper safety precautions for use

Intestinal obstruction- this is a known complication in patients with cystic fibrosis, therefore, if symptoms resemble this condition, it is necessary to remember about possible intestinal strictures. The drug contains active enzymes that can damage the mucous membrane oral cavity, up to the formation of ulcers, so the tablets must be swallowed whole without chewing. The drug should not be used in patients with rare hereditary forms galactose intolerance, lactase deficiency or glucose-galactose malabsorption syndrome.

Use during pregnancy or breastfeeding

During pregnancy and breastfeeding, take the drug as prescribed by a doctor if the expected benefit to the mother outweighs the potential risk to the fetus/child.

The ability to influence the reaction rate when driving vehicles or other mechanisms

There are no reservations.

Children

Use for children aged 3 years and older.

Directions for use and doses

The dose of Mezim â forte 10000 depends on the deficiency of pancreatic enzymes in duodenum and is installed individually. If there are no other recommendations and in cases of consumption of difficult-to-digest plant food, fatty or unusual foods, take 1-2 tablets. In other above-mentioned cases, when digestive disorders occur, the recommended dose is 2-4 tablets. If necessary, the dose can be increased. Increasing the dose to reduce symptoms of the disease, such as steatorrhea or abdominal pain, should only be done under the supervision of a physician. The daily dose of lipase should not exceed 15,000-20,000 IU E.F. per 1 kg of body weight.

Take the tablets with meals, swallow them whole with plenty of liquid, such as a glass of water.

The duration of treatment depends on the course of the disease and is determined by the doctor individually.

The question of the dosage of the drug and the duration of treatment for children is decided by the doctor.

Mezim ® forte 10000 should be prescribed in the daily dose required to normalize stool, but not more than 1500 IU of lipase per 1 kg of body weight of a child under 12 years of age. Children over 12 years old daily dose enzymes should not exceed 15,000-20,000 IU E.F. lipase per 1 kg of body weight.

Overdose

Strengthening possible side effects. During administration, it is extremely high doses Hyperuricemia and hyperuricosuria have been observed with other pancreatic powder preparations. Treatment is symptomatic.

Side effects

To assess the frequency of side effects, the following gradation is used: very often ≥ 1/10; often: ≥ 1/100 and<1/10; иногда: ≥ 1/1000 и <1/100; редко ≥ 1/10000 и <1/1000; очень редко <1/10000, частота неизвестна (оценка не может быть проведена по имеющимся данным).

From the cardiovascular system. Frequency unknown: tachycardia.

From the immune system. Very rarely, immediate allergic reactions (skin rash, sneezing, lacrimation, bronchospasm). Frequency unknown: Azorubic varnish (E 122) may cause allergic reactions.

From the digestive tract. Very rarely, in patients with cystic fibrosis, when using high doses of pancreatin, narrowing may form in the ileocecal region and in the ascending colon; allergic reactions from the digestive tract (diarrhea, abdominal pain, nausea, change in stool character).

The main organ in the human digestive system is the stomach - the main process of food digestion takes place here, in which many enzymes take part. Their deficiency provokes unpleasant sensations: pain, a feeling of heaviness, heartburn. You can get rid of them by buying Mezim at the pharmacy.

1 Registration number and ATX

In the anatomical-therapeutic-chemical classification it has code A09AA02 - multienzyme preparations. Registration number - No. 14681/01.

On the shelves of pharmacies you can find Mezim, the name of which has the word “forte” added to it. It means that the tablets are in an enteric coating, which protects the enzymes they contain from loss or reduction in activity.

2 Indications for use

You can take Mezim:

  • if you have to do an ultrasound of the abdominal organs (usually the drug is taken the night before);
  • if you have chronic diseases of the gastrointestinal tract, liver and gallbladder;
  • there are gastrointestinal disorders;
  • if you have insufficiency of the exocrine function of the pancreas (chronic pancreatitis, cystic fibrosis, etc.);
  • when it is necessary to restore the gastrointestinal tract after radiation therapy or removal of an organ;
  • if it is necessary to prepare for an x-ray examination;;
  • after overeating or regular poor nutrition.

3 Pharmacological action

Mezim is an enzyme drug that helps the gastrointestinal tract function. The main medicinal substance in it is pancreatin, which contains lipases, amylases, proteases, etc.

Pharmacodynamics: pancreatic enzymes promote the breakdown of essential nutrients, which are subsequently more easily absorbed in the small intestine. Trypsin in the composition relieves pain. The effect is most felt after 30-45 minutes.

Pharmacokinetics: the acid-resistant shell dissolves only in a neutral or slightly alkaline environment, therefore, despite the influence of stomach acid, enzymes are preserved and activated in the intestine.

If Mezim tablets do not have a protective film, then they can be used as an anesthetic for chronic pancreatitis.

4 Composition and release form

Pink round tablets.

1 pill contains the active ingredient - 137.5 mg of pancreatin. There are excipients: lactose, cellulose, silicon dioxide, etc.

Important! Pancreatin is not a monosubstance. It consists of lipase, protease and amylase. The first is involved in the breakdown of lipids, the second - proteins, the third - carbohydrates.

5 How to take?

Dosage

You can use the instructions without a doctor's prescription. According to it, adults can take from 2 to 4 tablets at a time. In children, the dose is lower - no more than 500-20,000 Ph units. Eur. lipase per kilogram of body weight.

Before or after meals?

The tablets should be taken as follows:

  • 1/3 or 1/2 of a single dose as soon as you start eating;
  • the rest - in the middle of the meal.

It is recommended that you swallow the tablet while sitting or standing to prevent the pill from sticking to the esophagus, where premature release of digestive enzymes may occur. After taking it, do not lie down for at least 5 minutes. In addition, by sticking to the esophagus, the drug can damage its mucous membrane.

Duration of use

The duration of treatment depends on the prescription of the gastroenterologist or therapist. This could be several days, months or years.

If taking Mezim is caused by digestive disorders as a result of overeating or eating unhealthy food, then you cannot save yourself with the drug every time for a long time. Firstly, this will cause addiction to the drug, and secondly, the secretion of the pancreas decreases and it stops producing its enzymes.

For the treatment of chronic gastrointestinal diseases, lifelong use of the drug is possible. For example, with flatulence, gallbladder diseases. You will have to use Mezim after some surgical operations; in case of obstruction of the bile duct, its perforation; inflammation or injury to the pancreas. Then Mezim is prescribed for manifestations of dyspepsia.

6 Features of application

Pregnancy and lactation

During pregnancy, a peculiar restructuring occurs in a woman’s body, often accompanied by unpleasant sensations, including from the gastrointestinal tract. The enlarging uterus puts pressure on the internal organs and interferes with the normal processes of food digestion.

Pregnant women and women whose children are breastfed can take Mezim, but only with a doctor's prescription. If there is a deficiency of enzymes or their improper absorption, the drug is prescribed, since the lack of nutrients leads to improper development of the child.

Childhood

Due to their age, children, especially young ones, cannot independently determine their food culture. In addition, a child, while learning about the world around him, puts foreign objects into his mouth, which are not always clean. Dirt also enters the body through unwashed hands. Children sometimes eat excessively fatty or fried foods. All this can provoke digestive disorders, accompanied by:

  • nausea and vomiting;
  • diarrhea or constipation;
  • flatulence, seething, pain;
  • loss of appetite and weight loss;
  • motor passivity.

To eliminate them, you can use Mezim. However, remember that use should not become regular as a panacea for any gastrointestinal disorders. If these pathologies are observed frequently, take your child to the pediatrician.

If children are under 12 years old, they are entitled to up to 1,500 lipase IU per 1 kilogram of weight, and older ones - no more than 20,000.

Important! Do not crush or mix the tablet in liquids. This will disrupt the integrity of the acid-resistant film, and the enzymes will not reach the intestines.

Elderly age

As a result of the aging of the human body, the capabilities of all organs, in particular the digestive tract, become limited. Involution of the gastrointestinal tract occurs slowly and is associated primarily with a decrease in the functional activity of the pancreas, which plays a major role in the breakdown of food.

Age-related destruction of the gland begins at the age of 40, but under unfavorable conditions it can occur earlier. Elderly and elderly people are often diagnosed with chronic pancreatitis with manifestations in the form of flatulence, pain, abnormal bowel movements, etc. To improve the condition, doctors prescribe digestive enzyme preparations.

The drug has shown good results in the treatment of chronic pancreatitis.

  1. Abdominal bloating decreases or disappears.
  2. Pain syndrome (abdominal pain) is relieved.
  3. The drug is well tolerated.
  4. With continuous use, a complete correction of maltigestion and malabsorption occurs.

Mezim should be taken 1-2 tablets 3 times a day with meals. 2 tablets are immediately prescribed for severe pain to speed up its relief.

For liver dysfunction

Mezim is not a specific medicine for liver cirrhosis, but is used as part of a combination treatment.

This serious disease is accompanied by dysbiosis and deficiency of pancreatic enzymes. Flatulence, pain, and a feeling of inoperative gastrointestinal tract are recommended to be relieved with the help of enzyme preparations. Festal is sometimes taken, but it contains bile acids that accumulate in the body. Mezim does not have this feature and is no less effective.

For renal dysfunction

Kidney diseases are sometimes accompanied by gastrointestinal reactions such as nausea and vomiting. However, as part of complex therapy, Mezim is not used to relieve these symptoms, since they have a different etiology that has nothing to do with a violation of the enzymatic ability of the pancreas.

7 Side effects

For some diseases, Mezim is taken for a long time, and no side effects have been recorded. Theoretically, it is possible to develop allergic reactions to the components of the drug, rarely - diarrhea, constipation or discomfort in the abdomen.

In some patients with cystic fibrosis, when taking large doses of the drug for a long time, hyperuricemia and narrowing of intestinal sections (formation of strictures) may develop.

Effect on driving

Mezim does not affect the driver’s reaction speed and his perception or assessment of the situation, therefore, taking this drug to people whose profession is associated with driving vehicles or with complex mechanisms does not pose a danger.

8 Contraindications

Mezim in any dosage of pancreatin is contraindicated if you have:

  • hypersensitivity;
  • hereditary hypolactasia;
  • there is not enough lactase or there is glucose-galactose malabsorption syndrome;
  • acute pancreatitis;
  • exacerbation of chronic pancreatitis;

Important! The drug should not be given to children under 3 years of age.

9 Interaction with other drugs

If you take an iron supplement, then in tandem with Mezim they will not be fully absorbed, which will cause anemia. Temporarily exclude Mezim from your therapy or the doctor will select another medicine.

When taking pancreatin, folic acid is poorly absorbed and the effect of glucose-lowering drugs worsens.

Antacids reduce the effectiveness of Mezim.

10 Compatibility with alcohol

Mezim can improve digestion not only during a hangover, but also during a feast. With moderate alcohol consumption, the drug accelerates the breakdown of ethanol, thereby reducing its harm to the liver, kidneys and brain. The drug begins to act within half an hour after administration, at the same time the person who has consumed alcohol begins to sober up.

With constant use of Mezim, drinking alcohol reduces the effectiveness of the drug. The harm from such a tandem is considered minimal, compatibility is relative.

11 Overdose

There is no need to interfere with this process. This is how the body tries to get rid of excess drug on its own. If severe nausea is observed, but there is no vomiting, it can be induced artificially, then give the victim plenty of fluids, a diuretic or laxative.

Sometimes an overdose occurs due to long-term use of the drug, for example, in the treatment of chronic pancreatitis, which can lead to hyperuricemia or hyperuricosuria. In such cases, Mezim is canceled.

In children, an overdose may cause constipation.

Pharmacodynamics. An enzyme preparation that improves digestion.
Pancreatin is a powder from pork pancreas, which, along with the exocrine pancreatic enzymes - lipase, amylase, protease, trypsin and chymotrypsin, also contains other enzymes.

Pancreatic enzymes included in the drug facilitate the breakdown of proteins, fats and carbohydrates, which leads to their more complete absorption in the small intestine. Trypsin suppresses stimulated pancreatic secretion, producing an analgesic effect. The maximum enzymatic activity of the drug is observed 30-45 minutes after oral administration.

Pharmacokinetics. Mezim ® forte 10000 tablets are coated with an acid-resistant coating that does not dissolve under the action of stomach hydrochloric acid and thereby protects the enzymes contained in the drug from inactivation. Dissolution of the shell and release of enzymes occurs at a pH value close to neutral or slightly alkaline.

Indications for use

- replacement therapy for exocrine pancreatic insufficiency (including chronic pancreatitis, cystic fibrosis);
- chronic inflammatory-dystrophic diseases of the stomach, intestines, liver, gall bladder;
- conditions after resection or irradiation of the gastrointestinal tract, accompanied by impaired digestion of food, flatulence, diarrhea (as part of combination therapy);
- gastrointestinal disorder of a functional nature (with intestinal infectious diseases, irritable bowel syndrome, etc.);
- to improve food digestion in patients with normal gastrointestinal function in case of errors in nutrition;
- preparation for x-ray and ultrasound examinations of the abdominal organs.

Use during pregnancy and lactation

Due to the lack of sufficient data on the use of pancreatic enzymes in humans during pregnancy and lactation, the use of Mezim forte 10000 is possible only if the expected benefit to the mother outweighs the possible risk to the fetus or child.

Directions for use and doses

The dose of Mezim ® forte 10000 is set individually depending on the severity of the disease and the composition of the food.

Unless otherwise indicated, the average single dose for adults is 2-4 tablets of Mezim ® forte 10000 per meal. It is recommended to take half or a third of a single dose at the beginning of a meal, and the rest during it. The drug is taken orally, without chewing and with a sufficient amount of liquid.

It is possible to increase the dose, which should only be carried out under the supervision of a physician, focusing on the weakening of symptoms (for example, steatorrhea, abdominal pain).

The maximum daily dose is 15,000-20,000 Ph units. Eur. lipase/kg body weight.

For children, the dosage regimen is determined by the doctor depending on the severity of the disease and the composition of the food at the rate of 500-1000 Ph units. Eur. lipase / kg of child’s body weight for each meal.

The duration of treatment can vary from several days (for indigestion, dietary errors) to several months and even years (if constant replacement therapy is necessary).

Side effect

No side effects or complications were detected even with long-term and regular use of Mezim® forte 10000 in patients with impaired pancreatic function.

In some cases, after taking pancreatin, allergic reactions may develop, rarely - diarrhea or constipation, nausea, discomfort in the epigastric region. In isolated cases, in patients suffering from cystic fibrosis, with prolonged use of high doses of the drug, hyperuricosuria (increased levels of uric acid in the blood plasma) may develop, and strictures may form in the ileocecal region and ascending colon.

Overdose

There are no data on cases of overdose and intoxication with the drug.
Possible: hyperuricosuria, hyperuricemia, in children - constipation.
Treatment: drug withdrawal, symptomatic therapy.

Interaction with other drugs
When taking medications containing pancreatin, the absorption of folic acid may be reduced. The effect of hypoglycemic drugs (acarbose, miglitol) may decrease when taken simultaneously with pancreatin.
With the simultaneous use of pancreatin with iron preparations, the absorption of the latter may be reduced. The simultaneous use of antacids containing calcium carbonate and/or magnesium hydroxide may lead to a decrease in the effectiveness of the drug.

special instructions
In case of acute pancreatitis or exacerbation of chronic pancreatitis (at the stage of attenuation of exacerbation), during a restorative diet, it is advisable to prescribe Mezim forte 10000 against the background of existing or remaining insufficiency of pancreatic function.
Mezim ® forte 10000, taking into account the solid indivisible dosage form, is contraindicated in children under 3 years of age.
